Bring a photocopy of the following documents for evaluation.

i.     Learner Enrollment and Survey Form
i.     PSA Birth Certificate
iii.   PECD Checklist (for incoming Pre-Kindergarten 1 and 2)
iv.   SF 9/Report card from previous school
      (for incoming Kindergarten to Grade 6 students)
v.    2 pcs. ID photo (white background)
